Coaching Consent Form
Description of Coaching
Coaching is a process in which the coach and client discuss a variety of issues and experiences to increase the client’s personal awareness, generate insights, and encourage growth. Coaching is a comprehensive process that may involve many areas of one’s life including work, relationships, education, health, and more. The coach often helps the client set and achieve goals. Coaching can provide support, skills, accountability, and strategies for the client in order to achieve goals and solve problems in their life.
As your coach, my goal is to help you find passion and purpose, discover and align with your values, and help you achieve your full potential in a range of areas in your life.
Difference Between Coaching and Other Services
Coaching is not psychotherapy or mental health treatment. The coach does not diagnose, assess, or treat mental illness. Coaching is not marriage and family therapy. Coaching is not a substitute for mental health care, psychotherapy, or substance abuse treatment.
Nothing that the coach says should be interpreted as a reason or excuse to avoid or delay mental health or medical treatment. The services provided are supplemental to any treatment you, the client, are undergoing or will undergo in the future. Use of the word “healing” does not imply diagnosing or treating any conditions.
Coaching is present and future-focused. We may discuss the past when the client finds it helpful in dealing with the present and future.
The coach is not a substitute for the professional advice of a legal, medical, financial, or other qualified professional. Decisions in all areas of the client’s life are the client’s sole responsibility.
Role and Responsibility of Client
The benefits obtained from coaching depend on how well the client takes advantage of what they learn in the sessions. Deciding how to handle all life issues and utilize coaching in those areas is exclusively the responsibility of the client. While the coach will strive to assist the client in creating positive change, the coach assumes no responsibility for any outcomes the client may experience.
As the client, you understand that decisions made based on input from the coach are entirely your responsibility. The coach will not be liable for any actions taken or not taken by the client.
At times, coaching may be challenging, and difficult feelings may surface. The coach may question the client's assumptions and beliefs. The client is fully responsible for their own wellbeing, including during coaching sessions. The client is encouraged to provide feedback to the coach about what they like, don’t like, what is working, and want within the relationship. The coach agrees to be highly responsive to this.
If you are under age 18, the coach must receive written permission from your legal guardian in order to proceed with coaching.
Termination of Coaching Relationship
Both the coach and client have the right to terminate the relationship at any point in time, at their discretion. Reasons why the coach might terminate the relationship can include conflicts of interest, the coach believing they are not a good fit for the challenges presented by the client, the client appearing to not be benefitting from coaching, and more.
Fees and Payment
The coach reserves the right to periodically raise fees at his discretion.
Payment is required upon booking the session.
Refunds for cancelled appointments will exclude any transaction fees involved.
Confidentiality
The coach agrees to maintain confidentiality with regard to all information shared by the client, except where required by law including, but not limited to:
Reporting elder or child abuse
If the client suggests that they may be a significant danger to themselves or others
As required by legal proceedings
The client should note that confidentiality within a coaching relationship is not privileged under the law and information revealed by the client could become discoverable.
If the client requests or consents to the release of confidential information, including their name, the coach agrees to only do so if there is written authorization from the client.
On occasion, certain topics may be hypothetically and anonymously shared for educational, training, or consultation purposes. The coach agrees to change any identifiable details to protect the privacy of the client.
Security of Communication and Client Data
For any online communication between coach and client, including sessions or email exchanges, please be aware that it is impossible to maintain complete information security. Neither the coach nor client will be liable for the release of information arising from a security breach of any of our online communication.
The coach may take notes on the session. The coach agrees to maintain these in a secure location. However, if there is a breach of this information due to the unauthorized actions of a third party, the coach will not be held liable.
Governing Law and Jurisdiction
This coaching agreement is governed by the laws of California, United States. Any disputes arising under or related to this agreement will be resolved exclusively in the courts of California, United States. By signing, the client consents to the exclusive jurisdiction of these courts, regardless of their location.
Acknowledgement
By booking a session, you, the client, confirm that you have read, understand, and agree to all the terms and conditions listed above, and you provide informed consent to participate in coaching services.
